Teacher and Student Success Plan 2021-22

Name of Administrator:  Heather Hales

Name of School:  Country View Elementary School

Goal #1:

Country View Elementary is focused on creating a safe, nurturing, and engaging environment so that all students can access learning at high levels.  To better provide students with the social-emotional support they need to better access academic learning, student social-emotional wellness will be assessed and strengthened through instruction and supported by a schoolwide positive behavior plan.  In addition, school aides and a mental health specialist will provide time to support individual student behavior and mental health needs.

For student behavior, our goal is that the total of 2021-22 behavior office referrals will decrease by 10% from last year’s 2020-21 total.

For student emotional and mental wellness, our goal is that student sense of belonging will increase by 10% on the Panorama Student Social Emotional Learning Competency Survey from BOY to EOY.

Behavior office referrals are documented on MyStudent Tracker.  These Tracker incidents are tallied each quarter and at year end.  We will compare the total number of 2021-2022 Tracker incidents to the total number of 2020-2021 Tracker incidents to determine if a decrease of 10% was made.

Students in grades 3-6 will be given an emotional and mental health skills assessment provided by Panorama Education at the beginning of the year and at the end of the year.  We will analyze the Panorama EMH survey data to determine if student sense of belonging improved by 20% from BOY to EOY survey administration.
