Do you have an elementary-aged child who will be new to Country View Elementary next year?

  •  An incoming Kindergarten Student?
  •  A new student attending Country View Elementary?

Registration and Demographic Forms to Complete

If so, please go to http://wsd.net/parents-students/registration/school-registration to download, print, and complete your registration and demographic forms prior to New Student Registration.  There are seven documents to download and complete.  You may also contact the school office at 801-395-5650 to arrange to pick up a copy of the forms.

Please bring with you:

  • Original birth certificate
  • immunizations
  • two proofs of residency. (current utility bill, updated driver's license, W2 Form, etc.)
